Funny, they must have copied their forms from the official policy at ACC here in NZ
It looks about as useful as something a government department would come up with.

Hey, Bree! I work for an insurance company, as a call center girl answering questions for doctors and facilities. The number we refer people who need to talk to Medco Health is 1 800 251-7690.
As for a claim; I don’t know about Medco but at my company, the people who answer questions for members are not the same people who know anything about claims. I suggest you set aside an afternoon, call the toll-free number and be patient as they transfer you around. Ask about consideration time, turn-around-time (TAT), and appeals processing procedures so they know you are smart and mean business.

I have medco insurance and it took me 7 weeks and probably 15 or more calls to my Doctor the pharmacy and Medco to get one of my prescriptions covered.
To Duchess above, I was very proud of myself that I didn’t yell once at some poor innocent in the call center.
